Number 6 is a big one. For instance, why do so many registration forms ask for gender? It's strange how little attention these essential introductions to an app will receive!

Very helpful article. #9 seems to be mentioned a few times in comments: concern about how "secure" the website's infrastructure is (ala, "how safe is my password if I give it to you?"). Unfortunately, a lot of regular folks still use the same password for everything -- and think twice before signing up to anything.
